ºìÐÓÖ±²¥app Health System offers a network of Rehabilitation Outpatient Clinic sites conveniently located throughout the Laurel Highlands ºìÐÓÖ±²¥app. In addition to traditional physical, occupational, and speech therapies, unique programs are offered including, but not limited to, hand therapy, aquatic therapy, driving evaluations, pediatric therapy, and treatment of lymphedema.

Participation in an outpatient rehabilitation program means the patient may continue to stay at home and maintain an independent lifestyle. During treatment, the patient will learn how to reach maximum potential in regaining function, strength, mobility, self-care, endurance, or balance. An individualized care plan is created for each patient, and specifically designed to meet the needs and goals of that patient after injury, illness, or any other disabling event has occurred. ºìÐÓÖ±²¥app Rehabilitation Outpatient Clinic therapists work closely with ºìÐÓÖ±²¥app’s Corporate Care Services providing industrial rehabilitation, work site evaluations, functional agility, and functional capacity evaluations.

Referrals

Referrals to a ºìÐÓÖ±²¥app Rehabilitation Outpatient Clinic can be made by a physician, social worker, case manager, home health agency, or insurance company. Patients and their loved ones may also make a referral. A physician’s order is required before treatment can begin. To make a referral call (814) 266-4700.

Patient Information

Patients should arrive 15 minutes before the first appointment to register. Patients must bring proof of insurance, all assistive devices, and any adaptive equipment. Patients should also wear comfortable exercise clothing and shoes. If a patient is unable to schedule an appointment due to busy schedules, special arrangements can be made by calling the outpatient site.
